Together, their hard work and collaboration helped assure this patient got terrific care
Today we recognize great teamwork that pulled together a challenging discharge and helped assure an exceptional care experience.
Recently, we began caring for an out-of-state truck driver who experienced a serious healthcare incident while travelling through Battle Creek. Our medical care was complete and the patient ready to go home – but he was unable to drive, lacked resources, and had no friends or relatives who could help him make the trip. The situation was complicated by the need for a significant amount of portable oxygen.
Without getting into great detail, social work, case management, floor nurses, and physicians all worked together to craft a solution that helped this man travel across the country and back home.
Special thanks to MSW Rachel Owens; Denise Travis, Judy Travis and Rebekah Reed from case management; Drs. Rathburn and Sheldon from our hospitalist service; and the nursing staff and care team on C3. Together, their hard work and collaboration helped assure this patient got terrific care – and was delivered safely home after a life changing diagnosis.
