Recently, the hospital was caring for a very sick patient with significant medical and behavioral health challenges. Our C3 and Fieldstone care teams partnered beautifully with social work, case management, and security. Kudo’s to all involved for great communication, flexibility, creativity, and trusting relationships that provided a positive result.
